  • Email or username used to login with
  • School or entity the user account is aligned to
  • Office version(s) - i.e. 2016, 2019, Online
  • Course(s) to be reset - i.e. Word 2016 Core or Excel Associate Online
  • Optional, but helpful - Group Key(s) associated to the Course(s) to be reset
